Special Notice

The University of Connecticut has imposed a moratorium on the destruction of all financial records associated with federally supported projects until 2011.

Disposal Request Form

The RC-108 form (WORD, 106KB) is also available directly from the Connecticut State Library's website. Although it may be completed and submitted electronically to the University's Records Management Liaison (Unit 1205), a signed paper copy will be returned that must be retained permanently by the requesting office.



Shredding Service Request Form

Shredding is available for Confidential records by Central Stores staff. Once destruction has been approved, requests may be made to Central Stories with the submission of a Shredding Service Request Form. The state also maintains contracts with vendors for these services also. Please contact the Liaison for additional information.
